Read the Instructions
Carefully read the provided instructions. Once you are ready to apply, click the link that will take you to the Application Portal.

Create a New Account
On the Application Portal, click the “CREATE A NEW ACCOUNT” button.
Fill in the required personal details as an applicant.
Important: Your phone number, email address, or pincode will serve as your default password to log in to the portal.

Complete the Registration
Once your account has been created, click the “FILL APPLICATION FORM” button to proceed.

Fill the Application Form
Provide accurate information in the application form.
Ensure you carefully review each section before proceeding.

Upload Required Documents
Before submitting your application, you will need to upload the following documents:
Passport-sized photograph
Scanned PDF copy of your LGA Certificate
Birth Certificate
Government-issued ID Card (Driver’s License, Permanent Voter’s Card, International Passport, or NIN Slip)

Submit the Application
Once you have completed the form and uploaded all required documents, submit the application.
Upon successful submission, an email will be sent to you containing:
A completed copy of your application form
Your examination photocard

Edit Your Application (Optional)
If you need to make any changes, you can edit and resubmit your form before the application deadline. Simply log in to the portal, click on the “Edit Application Form” icon, make the necessary changes, and resubmit.
By following these steps, eligible participants can successfully apply to join the Obagi HCDT Empowerment & Skill Development Program.
